Welcome to the Team Dynamics Questionnaire.

Thank you in advance for your time and transparency. As you complete this questionnaire, please answer within the context of your participation within the team referenced in your instructions. We know that throughout your work functions you may work across teams and functions, but to get the most accurate results from this questionnaire, we ask that you focus only on the interactions that you have within this specified team.

Instructions: This questionnaire consists of 35 rating-scale questions and 5 open-ended text response questions. In total, it should take no more than 15 - 20 minutes to complete. Once completed, all responses will be aggregated and then a report will be provided to the designated Team leader for distribution.
